835 Exception Review Queue Template
Use this template when a remittance or posting item does not move cleanly through routine posting. The purpose is to organize exception review, not to automate payer, posting, coding, payment, or compliance decisions.
Exception review fields
| Field | Use |

| Batch/source category | Generic source label, not a public payer file or private identifier. |
| Exception cue | What made the item non-routine. |
| Posting lane | Routine, hold, follow-up review, documentation review, or supervisor review. |
| Blocker | Missing context, unclear adjustment cue, documentation need, or review dependency. |
| Owner/reviewer | Who gathers context and who approves the next step. |
